Partnership creates region’s largest nonprofit home care provider

Atrio Home Care and Brio Living Services have announced a strategic partnership that will align all home health care operations under Atrio Home Care. The partnership will expand access and strengthen service delivery across West Michigan.
Atrio Home Care operates as a collaboratively owned organization within the Trillium Living senior population health collective. Under a new ownership structure, Trillium Living will hold a majority stake, while Brio Living Services and Resthaven will each retain a 20% equity interest.
“We are committed to making this transition as seamless as possible for patients, families, and team members while continuing to deliver the high-quality care our communities depend on,” said Mike Hamilton, Executive Director of Atrio Home Care. “This partnership reflects a shared commitment to preserving access to exceptional nonprofit home health care in West Michigan.”
Patients currently receiving care through Porter Hills Home Health Care, operated by Brio Living Services, are not expected to experience disruption. Services will continue under Atrio Home Care, with additional details on staffing, referrals, and operations to be shared during the transition.
Atrio Home Care will serve as the operating organization for all home health services. The West Michigan provider currently serves 2,800 patients across Allegan, Barry, Ionia, Kent, Montcalm, and Ottawa counties. Approximately 22 employees will transition as part of the agreement, supporting continuity of care and day-to-day operations. With this partnership, Atrio Home Care will represent the last remaining non-hospital-based nonprofit home health care provider in West Michigan, reinforcing its long-standing commitment to mission-driven, community-centered care.

The partnership supports a broader effort to strengthen care coordination across Trillium Living’s network. Through its affiliated organizations, Trillium Living provides housing and services for older adults across West Michigan, from in-home care to higher levels of support. The transition is expected to begin in June 2026 and conclude by July 1, 2026.
