Partnership supports expansion of in-home care

Mitten Home Infusion, a Grand Rapids-based provider of concierge home infusion and mobile nursing services, is partnering with Michigan-based Coordinista for its field care delivery platform to support the coordination, scheduling, and operational logistics behind its growing in-home care model.
By implementing Coordinista, Mitten Home Infusion will equip its clinical and operational teams with technology designed specifically for organizations delivering care across decentralized locations.
Mitten Home Infusion provides skilled Registered Nurse staffing and personalized infusion therapy in patients’ homes, helping patients receive complex treatments in a comfortable and convenient environment. The organization supports specialty pharmacy patients receiving therapies including IVIG, SCIG, biologics, anti-infectives, teaches and trains, and other infusion services.

Coordinista is a cloud-based field management platform built for healthcare organizations operating decentralized care delivery models. The platform provides capabilities including caseload management, intelligent scheduling, patient and service management, geographic mapping and routing, mileage tracking, electronic visit verification, workflow automation, and operational reporting.
